LOLER - Reports and Defects
When the competent person has carried out an examination / inspection of an item of lifting equipment, a report of any defects found should be notified to the owner user of the equipment. A verbal notification should be given immediately, where the defect is serious or if it needs to be fixed before the equipment is used again. A follow up written report should then be forwarded within 28 days of the examination.

All defects should be reported even if they were rectified on the spot, because it could be hiding a more serious fault. The report should be in writing. The report may also be sent to relevant enforcing authorities particularly if the fault is a design fault and could produce serious injury.

Types of defects found which are commonly reported include:

Cracks and permanent deformation.

Corrosion of vital parts.

Excessive wear and tear or failure of moving parts.

Significant misalignment.

Lifting Accessories also need to be inspected / examined on a daily and annual basis. Common defects found may include:

Slings - damaged, cut, abraded or stretched.

Chains - deformed or have stretched links.

Wire ropes - broken strands of wire or the wire has kinks in it.
